Microsoft Forms is generally available in Office 365 Education.  Check to see if you have access to this app in your board.  You can also go to the Microsoft help centre for setting up a Form.

With Microsoft Forms, you can create surveys, quizzes, and polls, and easily see results as they come in. When you create a quiz or form, you can invite others to respond to it using any web browser, even on mobile devices.  The video below provides a basic introduction to Microsoft Forms.
